Eligibility
MassHealth dental coverage is available to Massachusetts residents who meet income, residency, and citizenship or immigration requirements. Eligibility can also depend on age, medical needs, disability status, pregnancy, or enrollment in programs such as the Department of Developmental Services (DDS). Please note that adults age 21 and older who are not DDS clients are subject to a $1,750 annual dental maximum per benefit year beginning August 1, 2026. Understanding Your Benefits
MassHealth dental benefits vary depending on your plan type and age. Children typically receive the most comprehensive coverage. Adults receive medically necessary dental care, subject to a $1,750 annual maximum per benefit year (effective August 1, 2026), while DDS members may qualify for expanded benefits with no annual maximum. Some services may require prior authorization before treatment.
$1,750 Annual Maximum for Adult MassHealth Dental
Effective August 1, 2026, MassHealth set an annual maximum of $1,750 per member per benefit year for adult dental benefits.
Who It Applies To
- Adults age 21 and older enrolled in MassHealth Standard, CommonHealth, Family Assistance, or CarePlus
- Adults whose dental services are paid by the Health Safety Net
Who It Does Not Apply To
- Members under age 21
- Adults age 21 and older who are DDS clients
- Members under 21 receive care through our Children's Dentistry services
How It Works
- The benefit year runs July 1 to June 30. The first benefit year runs August 1, 2026 to June 30, 2027.
- The maximum resets to $1,750 each July 1. Unused amounts do not roll over.
- The amount counted is what MassHealth actually pays for covered services, not our billed charge.
- Your balance does not reset if your coverage changes or lapses during the benefit year.
- If you turn 21 during the benefit year, the maximum applies from your 21st birthday through June 30.
- Covered services and benefit limits have not changed. Services that were not covered before, such as night guards, are still not covered.
When You Reach the Maximum
Adult patients with developmental disabilities often face dental challenges that a typical office visit doesn’t account for, such as the following:
- MassHealth stops paying for most covered services for the rest of that benefit year.
- Some services can still be covered, including limited dental exams, certain X-rays, denture repairs, tooth extractions, emergency pain relief, anesthesia for covered procedures, treatment of surgical complications, a first-ever complete denture after full-arch extractions, and continued orthodontic treatment for members who turn 21 during MassHealth-approved treatment.
- For a service that is no longer covered, we can charge you only if you choose to be a private-pay patient and sign a written agreement before the service is provided. We also offer flexible patient financing for treatment that is not covered.
- If the maximum is reached partway through a service, you can be billed only for the portion MassHealth did not pay, and only if you signed the agreement beforehand.

DDS (Department of Developmental Services) Patients
Adults enrolled in DDS programs may qualify for expanded dental benefits, often similar to coverage provided to children. This may include additional preventive, restorative, and medically necessary services. The $1,750 adult annual maximum does not apply to adults who are DDS clients. Prior Authorization
Certain dental services require MassHealth approval before treatment begins—especially orthodontics, periodontal therapy, major restorative procedures, and complex oral surgery. Our administrative team manages all prior authorization requests and keeps you informed throughout the process.
MassHealth Limited
MassHealth Limited covers same day emergency care only. That includes treatment to relieve severe pain, control infection, reduce swelling, or address conditions that pose a health risk. If you are enrolled in MassHealth Limited, contact our office and we will review which emergency services may be covered.
Children’s Medical Security Plan (CMSP)
Smiley Dental Waltham accepts CMSP, which provides dental coverage for eligible children up to a specific annual maximum based on the child’s plan tier. Covered services may include:

- Limited restorative procedures

Is there a yearly limit on MassHealth dental coverage for adults?
Yes. Starting August 1, 2026, MassHealth adult dental coverage has an annual maximum of $1,750 per benefit year for members age 21 and older who are not DDS clients. Members under 21 and adult DDS clients are not subject to the maximum.
When does the benefit year start and end? Does unused money roll over?

What if I turn 21 during the year?
The maximum does not apply before your 21st birthday. It applies to services from your 21st birthday through June 30, then resets July 1.
Will my maximum reset if my coverage changes?
No. If your coverage changes or you lose and regain coverage mid-year, your balance continues from where it was. It resets to $1,750 on July 1.
Will I be billed if MassHealth stops paying?
Only if you knowingly choose to be a private-pay patient and sign a written agreement before the service is provided. Without that agreement, we cannot bill you for a service that is non-covered because the maximum was reached.
Does the annual maximum change which services are covered?
No. Covered services and benefit limits are the same as before. The maximum only limits how much MassHealth will pay in total during a benefit year.
What dental services are covered for adults under MassHealth?
Crowns are covered when medically necessary, such as for teeth that are severely decayed or fractured. Prior authorization may be required. You can read more about crowns and bridges on our website.
Does MassHealth cover cosmetic dentistry or whitening?
Adults may receive exams, cleanings, X rays, fillings, extractions, dentures, crowns when medically necessary, root canals on certain teeth, and some periodontal services. Coverage varies by plan. Adult coverage is subject to a $1,750 annual maximum per benefit year (July 1 – June 30).
Are children fully covered under MassHealth?
Yes. Children under 21 receive comprehensive benefits, including preventive care, restorative services, fluoride treatments, sealants, X rays, fillings, extractions, certain periodontal procedures, and medically necessary orthodontics with prior authorization.
Are dentures covered under MassHealth?
Yes. Full and partial dentures are covered when medically necessary. Replacement dentures may require justification. A first-ever complete denture after full-arch extractions can be covered even if you have reached the annual maximum. Replacement dentures count toward the annual maximum.
Does MassHealth cover root canals on all teeth?
Coverage depends on the tooth. Front teeth and premolars are generally covered, while molars may require additional justification or may not be covered depending on the plan.
Will I have out-of-pocket costs?
Most medically necessary services are covered. Any non-covered services or optional upgrades may involve out-of-pocket costs, which we always explain in advance, and we offer flexible financing options for anything not covered. Adults who reach the $1,750 annual maximum may have out-of-pocket costs for services MassHealth no longer covers that year. We will explain this in advance and only proceed with a signed private-pay agreement.
